Thumb sucking with nose deformation and thumb atrophy in adult: A case report

The aim of this study report is to present nose deformation and thumb atrophy case that results from thumb sucking habit which has not been published previously in the literature. R.Ş. a 21 years old patient was admitted to the hospital with anxiety symptoms. He was diagnosed to have adaptive disorder and antisocial personality disorder according to DSM-IV criteria. His mother was diagnosed with schizophrenia while in pregnancy period. R.Ş. didn’t take breastfeeding because of the adverse effects of antipsychotic drugs. He has been sucking his thumb after 2 years old and this behavior has continued during the adult ages. Thumb sucking was related with emotional deprivation especially in increasing anxiety, fear, worry and stressful life events. It could reveal during the sleep latency and in the evening time, sometimes he indicates that he sleeps with thumb sucking. While hospitalized, he was consulted with ear-nose-throat and dental treatment departments. At paranasal sinus CT, the right side of frontal sinus, mucous membrane of the ethmoid cells were seen to become thick and nasal septum was seen as deviated to left side. After that, the patient was referred to Dentistry Department for breaking his habit. There was also a significant thumb atrophy at his left hand and palatal dome (0.8 cm). Finger sucking can result in deformities of the fingers as well as the deformities well-known in orthodontics. Besides skin changes resulting from chronic irritation, more serious deformities in position or function of fingers may occur. In our case, we also indicated the left hand thumb deformities, palatal doming, nasal septal deformities.


Burun deformitesi ve başparmak atrofisi ile giden parmak emme davranışı: Olgu sunumu

Bu olgu sunumunun amacı başparmak emme alışkanlığı nedeniyle parmak atrofisi ve burun deformitesi gelişen ve son zamanlarda literatürde hiç yayımlanmamış bir vakayı sunmaktır. Olgu, 21 yaşında erkek hasta olup anksiyete semptomları sergilemesi üzerine kliniğe kabul edildi. DSM-IV kriterlerine göre antisosyal kişilik ve uyum bozukluğu tanısı kondu. Gebelik döneminde annesine şizofreni tanısı konmuş idi. Antipsikotik ilaçların yan etkisi nedeniyle anne sütü ile beslenememişdi. İki yaşından sonra parmak emme davranışına devam etmiş ve yetişkin yaşlarda halen devam etmekte idi. Parmak emme özellikle duygusal yoksunluk, kaygı, korku, endişe ve stresin artış gösterdiği durumlar ile ilişkilidir. Uykuya dalma sırasında ve akşam saatlerinde artış göstermekte olup bazan uyurken parmak emdiğini belirtmektedir. Hospitalizasyon sürecinde kulak burun boğaz ve diş tedavi servisine konsülte edilmiştir. Paranazal sinüs tomografisinde frontal sinüsün sağ tarafı, etmoid hücre mukoz memranında artış ve nazal septumun sola deviye olduğu gözlenmiştir. Daha sonra hasta alışkanlık kırıcı davranış sağlanması amacıyla diş departmanına yönlendirilmiştir. Hastanın sol el başparmağında anlamlı atrofi ve damakta kubbeleşme tespit edildi (0.8 cm). Parmak emme davranışı parmak deformitesine neden olduğu kadar iyi bilinen ortodontik sorunlar da yol açabilmektedir. Kronik irritasyona bağlı deri renginde değişikliğin yanı sıra parmağın fonksiyonunda da önemli deformiteler geliştiği gözlenmektedir. Olgu sunumumuzda sol el başparmak atrofisi, damak kubbeleşmesi ve nasal septal deformitesini belirttik.
