The association of psychiatric symptoms and treatment of acne vulgaris with isotiretinoin

Objective: Isotiretinoin is a very effective drug against severe, stubborn acne. Several studies were performed evaluating the relation between isotretinoin with psychiatric side effects such as depression and suicidal ideation. However, the results are contradictory. The aim of this study is to follow whether depression, and suicidal ideas develops during isotretinoin therapy.

Method: 20 patients, who were admitted to dermatology outpatient clinic with complaints of acne. Isotretinoin therapy was initiated and patients were followed up for 16 weeks for psychiatric symptoms. Patients were applied SCID-I, Beck Depression Inventory, Beck Hopelessness Scale, Hamilton Anxiety Rating Scale, Hamilton Depression Rating Scale, and socio-demographic data were collected by semi-structured interview form.

Results: Except one of the patients under follow up, there was no observed depressive symptoms or suicidal idea. On the contrary, subthreshold depressive symptoms of patients, who benefited from treatment decreased. Depressive symptoms has initially declined, in one patient. One patient developed depressive symptoms.

Conclusion: In persistent acne patients, significant benefit of isotretinoin treatment was seen and there was no depressive symptoms, except one. In contrast, reduction in mean depressive symptom scores was determined. Reduction in depressive symptoms likely to have developed due to the secondary benefit of treatment.


Acne vulgarisin isotiretinoin ile tedavisi ve psikiyatrik belirtilerin birlikteliği

Amaç: İsotiretinoin şiddetli, inatçı aknelere karşı çok etkili bir ilaçtır. İsotretinoin kullanımının depresyon, suisid düşüncesi gibi psikiyatrik yan etkiler ile ilişkisi olduğuna dair çeşitli çalışmalar yapılmış ve sonuçları çelişkili bulunmuştur. Bu çalışmanın amacı isotretinoin tedavisi sırasında depresyon ve suisid fikirlerinin ortaya çıkıp çıkmadığını takip etmektir.

Yöntem: Dermatoloji polikliniğine akne şikayetiyle başvuran ve isotretinoin tedavisi başlanan 20 hastanın psikiyatrik belirtileri 16 hafta boyunca takip edildi. Hastalara SCID-I, Beck Depresyon Envanteri, Beck Umutsuzluk Ölçeği, Hamilton Anksiyete Ölçeği, Hamilton Depresyon Ölçeği uygulandı ve yarı yapılandırılmış görüşme formu ile sosyodemografik verileri toplandı.

Bulgular: Hastaların birisi haricinde takip sürecinde depresif belirtiye yada suisid düşüncesine rastlanmadı. Aksine tedaviden fayda gören hastaların eşik altı depresif belirtilerinde azalma tespit edildi. Bir hastanın başlangıçta var olan depresif belirtileri geriledi. Bir hastada depresif belirtiler gelişti.

Sonuç: İsotretinoin kullanan inatçı akne hastalarının tedaviden anlamlı düzeyde fayda gördüğü ve birisi haricinde depresif belirtiye rastlanmadığı görülmüştür. Aksine hastaların ortalama depresif semptom skorlarında azalma tespit edilmiştir. Bu durumun muhtemel sebebi de tedaviden görülen faydaya sekonder gelişmiş olmasıdır.
