Although most patients with panic disorder readily respond to serotonin reuptake inhibitors, some patients are treatment resistant to standard pharmacological treatments. Switching to another antidepressant, drug combination and augmentation strategies were suggested to overcome the treatment resistance. There were few case reports in the literature adressing the usage of atypical antipsychotics in the treatment of resistant panic disorder patients. In the most of the cases, olanzapine were added to selective serotonin reuptake inhibitors. There is no report on augmentation therapy with quetiapine in patients with panic disorder. In this report we presented a patient with panic disorder and agoraphobia who responded well to quetiapine added to ongoing paroxetine treatment.
Panik bozukluğu tedavisinde ketiapin ile güçlendirme: Bir olgu sunumu
Panik bozukluğu hastaları genellikle serotonin geri alım inhibitörleri ile kolaylıkla tedavi edilebilmekle birlikte, bazı hastalar standart farmakolojik tedavilere dirençlidir. Tedaviye direnci aşmak için bir başka antidepresana geçmek, ilaç kombinasyonları ve güçlendirme stratejilerinin kullanılması önerilmektedir. Literatürde tedaviye dirençli panik bozukluğu hastalarının tedavisinde atipik antipsikotiklerin kullanımı ile ilgili az sayıda hasta bildirimi bulunmaktadır. Hastaların çoğunda serotonin geri alım inhibitörlerine olanzapin eklenmiştir. Panik bozukluğu hastalarında ketiapinin güçlendirme tedavisiyle ilgili bir bilgi yoktur. Bu yazıda paroksetin kullanan ve ketiapin güçlendirmesine iyi cevap aldığımız bir agorafobili panik bozukluğu hastası sunulmuştur.
