Pharmacotherapy and EMDR in the treatment of post-traumatic stress disorder: Methodological difficulties in comparing effectiveness

EMDR (Eye Movement Desensitization and Reprocessing) is a type of psychotherapy that involves activation of information processing capacity of the brain with its bilateral stimulation (eye movement, touch, sound). Its effectiveness in the treatment of Posttraumatic Stress Disorder (PTSD) has been previously observed and as a consequence of the following effectiveness studies the obtained evidence has lead its introduction to some prestigious guidelines like NICE and APA guidelines. However, available findings in the literature are still confusing. For instance, EMDR is identified to be effective in both acute and chronic PTSD patients and suggests recommending a trauma focused psychotherapy like EMDR and Cognitive Behavioral Therapy to those patients. On the other hand, experts who put these suggestions to these guidelines were most probably aware that there is no evidence based controlled study showing protective effects of EMDR against PTSD. Nevertheless, diverse conclusions of expert meetings achieved after evaluating nearly the same studies may be accepted as normal since the diagnostic validity of PTSD is a controversial topic. Issues like the primary symptom clusters when making diagnosis, the natural course and prognosis, treatment response to any kind of treatment, comorbid conditions, definitions of traumatic conditions and their types, and may be more importantly the gold standard of the treatment are ambiguous. Additionally, methodological differences, diversity of measurement instruments, differences in the sample itself and its size, and different targets of outcome measurements are the reasons that make harder to conclude treatment effectiveness.


Travma sonrası stres bozukluğu tedavisinde farmakoterapi ve EMDR: Etkinlik karşılaştırmasında metodolojik zorluklar

EMDR (Eye Movement Desensitization and Reprocessing, Göz Hareketleriyle Duyarsızlaştırma ve Yeniden İşlemleme) çift yönlü uyarımla (göz hareketleri, dokunma, ses) beynin bilgi işlemleme kapasitesinin harekete geçirildiği bir psikoterapi çeşididir (Shapiro,F.,1995). EMDR’ın Post Travmatik Stres Bozukluğu (PTSB) tedavisinde etkinliği gözlenmiş ve sonrasında çeşitli etkinlik araştırmaları neticesinde NICE ve APA gibi prestijli tedavi kılavuzlarına girecek düzeyde bilimsel kanıtlara erişilmiştir(Christopher WL, Schubert S.,2009). Buna rağmen bu konudaki literatürden elde edilen bulguların yorumlanması zor ve kafa karıştırıcı olmaktadır (Perkins & Rouanzoin, 2002). Sözgelimi, NICE ve APA tedavi kılavuzlarında EMDR’nin hem akut hem kronik PTSD’de etkin olduğu belirtilmekte olup, bu süreçteki kişilere EMDR veya Kognitif Davranışçı Terapi gibi travma odaklı psikolojik tedavilerden birinin yapilmasi tavsiye edilmektedir. Buna mukabil bu kılavuzlara bu öneriyi koyan uzmanlar EMDR’nin PTSB’ye karşı koruyucu etkinliğini kanıta dayalı olarak gösterebilecek kontrollü çalışmaların mevcut olmadığının da farkındadırlar. Aslen uzman uzlaşı gruplarının neredeyse aynı çalışmaları temel alarak yaptığı araştırma sonuçlarının yorumlanması işleminde farklı sonuçlara varması da normal kabul edilmelidir. Çünkü öncelikle PTSD tanısal geçerliği tartışmalı bir bozukluktur. Tanı koymada hangi semptom kümesinin önceleneceği,tabii seyir özellikleri ve prognozu herhangi bir tedaviye cevap biçimi komorbid durumların varlığı, travmatik olarak nitelenen durumları tanımlamada belirsizlik ve travmatik olayların çeşidi, daha da önemlisi tedavi söz konusu olduğunda ortaya konacak altın standardın ne olacağı gibi konular muğlaktır. Bunun dışında araştırma dizaynlarının farklılığı, ölçüm için kullanılan enstrümanların çeşitliliği, seçilen örneklem ve büyüklüğündeki farklılıklar ve sonlanım ölçütlerindeki farklı hedeşer tedavi etkinliği adına bir sonuca varmayı zorlaştıran diğer sebeplerdir (Maxfield &Hyer2002).
