The aim of the study was to discuss under the light of literature, the pathway of arginine-NO and the role of agmatine within the context of bipolar disorder with psychotic features through a case diagnosed as being in a manic episode with psychotic features thought to be induced by arginine-lysine- ornithine combination. The case, who is 21-year-old woman applied to hospital because of flight of ideas, paranoia and decrease in sleep. The case, who reported that she has been using arginine, ornithine and lysine supplement hoping to get taler, for 1 month. Also, she reported that the use of the aminoacid supplement and the occurence of the symptoms were concurrent. Through psychiatric examination, it was observed that her self-care was fine, her dressing was exaggareted when considered to her age; affect was elevated; and the speed of speech as well as the psychomotor activity were slightly increased. Within the context of thought, reference and persecutive delusions, erotomanic ideas were determined. Young Mania Rating Scale score was rated as 30. An organic pathology was not found through EEG, CBC and biochemistry tests. The case was prediagnosed as manic episode with psychotic features. Use of argenine, lyzine and ornithine supplement was stopped. Ketiapine 300mg/day was started, and, symptoms were found to be regressed after 1 month. Until now, there has been no case reported, which is related to manic episode with psychotic features, induced by aminoacid supplement. Despite there were cases reported in associaton with herbal products such as physalis, St John’s wort or ma huang use. The presence of the cases with diagnosis of manic episode induced by different types of herbal products, the determination of arginine (ADMA) in acut manic phase and the relationship of NO and agmatin with psychotic features led us consider that there could be a relationship between arginine pathway as well as NO and manic episode with psychotic features.
Arjinin, ornitin ve lizin kullanımının indüklediği manik hecme
Arjinin, ornitin ve lizin aminoasit takviyesi kullanımına bağlı eş zamanlı psikotik özellikli manik hecme geçirdiği düşünülen vaka üzerinden bipolar bozukluk patogenezinde arjinin-nitrik oksit (NO) yolağı ve agmatinin olası rolünü literatür eşliğinde tartışmayı amaçladık. Olgu, yirmi bir yaşında, kadın, üniversite öğrencisi olup, düşüncelerini toparlayamama, şüphecilik, uykusunda azalma şikayetleri ile ailesi eşliğinde polikliniğimize başvurdu. Son 1 aydır boy uzaması amacı ile arjinin, ornitin ve lizin takviyesi kullandığı öğrenilen hastanın şikayetlerinin başlamasıyla eş zamanlı olarak bu ürünü kullandığı anlaşıldı. Psikiyatrik muayenesinde; özbakımı iyi, giyimi yaşına göre abartılı, çok renkli, duygulanımı eleve, konuşma hızı ve psikomotor aktivitesi hafif artmıştı. Düşünce içeriğinde ise, referans ve perseküsyon sanrıları, erotomanik düşünceler saptandı. Klinisyen tarafından uygulanan Young mani derecelendirme ölçeğine (YMDÖ) göre 30 puan aldı. Yapılan EEG, tam kan ve biyokimya tetkikleri sonucunda organik patoloji saptanmadı. Psikotik özellikli manik hecme ön tanısı konan hastanın kullanmakta olduğu arjinin, ornitin ve lizin takviyesi kesildi. Ketiapin 300mg/gün başlanan hastanın 1 ay sonraki kontrolünde semptomlarının gerilediği saptandı. Bildiğimiz kadarı ile arjinin gibi aminoasit takviye ürünleri ile indüklendiği bildirilmiş psikotik özellikli manik hecme vakası yoktur. Ancak altın çilek, sarı kantoron otu (Hypericum perforatum), ma huang (Ephedra sinica) gibi bazı bitkisel preparatların kullanımı ile manik hecme geçiren olgu bildirimleri bulunmaktadır. Çeşitli bitkisel besin takviyesi preparatların, manik hecmeyi indüklediğini bildiren vakaların olması ve akut mani hecmesinde asimetrik dimetil arjinin seviyelerinin (ADMA) yüksek bulunması, NO ve agmatinin psikotik belirtiler ile ilişkili olması; arjinin-NO yolağının psikotik özellikli manik hecme arasında bir ilişki olabileceği varsayımını akla getirmiştir.
