Mood disorders Bipolar disorder and obsessive compulsive disorder comorbidity: a case report

Obsessive-compulsive disorder (OCD) and the comorbidity of major depressive disorder (MDD) is a common condition. Comorbidity of bipolar disorder (BD) and OCD is another important clinical manifestation. In literature studies reported that lifetime prevalence of OCD and bipolar disorder comorbidity was 12.4%, mostly with BP-II disorder. Latest analyses suggested that when comorbidity occurs with OCD and bipolar or unipolar depression, it has a differential impact on the clinical image and the course of OCD. In treatment of patient with OCD-BD comorbidity, the usage of mood stabilizer and cognitive behaviour therapy (CBT) is common in literature . A 43-year-old female, housewife; whose symptoms were anhedonia, distress, psychomotor retardation and depressive symptoms, was admitted to our clinic. Her complaints were having obsessive thoughts and compulsive behaviours for 30 years. She has been suffering from compulsive behaviours such as washing, counting and religious continuously. During her follow up period; she was given escitalopram 20 mg / day and aripiprazole 15 mg / day for depressive and obsessive symptoms however her respond to treatment was not enough. We internalized the patient for treatment but in the evolution of our patient we observed two hypomanic episodes thus we added valproic acid 1000 mg/day with the diagnosis of bipolar affective disorder. We have found that a significant decrease in the patient’s depressive symptoms. In this article we wanted to discuss and show the importance of BD in patient with OCD and depression. New treatment strategies must be improved for these cases and the clinicians must pay attention on these cases displaying hypomanic/manic episodes.
