Hormonal response to a single dose escitalopram administration in patients with panic disorder and generalized anxiety disorder

Objective: The aim of this study was to examine serotonergic system activity in patients with panic disorder and generalized anxiety disorder by investigating the responses of cortisol, dehydroepiandrosteronesulfate, and prolactin to escitalopram challenge. Method: Serum concentrations of cortisol, dehydroepiandrosteronesulfate, and prolactin were measured at baseline and after escitalopram administration in patients with panic disorder (n=24) and generalized anxiety disorder (n=21), and in healthy controls (n=20). Results: Hormonal responses to escitalopram administration in the patients with panic disorder and generalized anxiety disorder were found similar to those of the control group. The baseline dehydroepiandrosterone-sulfate levels were lower in patients with panic disorder and generalized anxiety disorder than those of the control group. Furthermore, dehydroepiandrosterone-sulfate levels were lower in women than those of men in both patients and control groups. Conclusions: There was no significant hormonal response to escitalopram in the patients and control groups, and there was no difference between the groups. The finding that basal dehydroepiandrosterone-sulfate level was lower in the patients with panic disorder and generalized anxiety disorder suggests that this hormone might play a role in the pathophysiology of these disorders.


Panik bozukluğu ve yaygın anksiyete bozukluğu olan hastalarda tek doz essitalopram uygulamasına hormonal yanıt

Amaç: Bu çal›flman›n amac› panik bozuklu¤u ve yayg›n anksiyete bozuklu¤u olan hastalarda essitaloprama kortizol, dehidroepiandrosteron-sülfat ve prolaktin yan›tlar›n›n ölçülmesi suretiyle serotonerjik sistem etkinli¤ini incelemek ve bu sistemin panik bozuklu¤u ve yayg›n anksiyete bozuklu¤unun patofizyolojisindeki rolünü araflt›rmakt›r. Yöntem: Panik bozuklu¤u (n=24) ile yayg›n anksiyete bozuklu¤u (n=21) olan hastalarda ve sa¤l›kl› kontrol (n=20 ) deneklerinde bazal ve essitalopram uygulamas› sonras›nda kortizol, dehidroepiandrosteron-sülfat ve prolaktin de¤erleri ölçüldü. Bulgular: Panik bozuklu¤u ve yayg›n anksiyete bozuklu¤u gruplar›nda essitalopram uygulamas›na hormonal yan›t kontrol grubuna benzerdi. Panik bozuklu¤u ve yayg›n anksiyete bozuklu¤u olan hastalarda bazal dehidroepiandrosteron-sülfat seviyesi kontrol grubuna göre düflüktü. Hem kontrol hem de hasta gruplar›nda bazal dehidroepiandrosteron-sülfat seviyesinin kad›nlarda erkeklerinkinden düflük oldu- ¤u bulundu. Sonuç: Hastalarda ve kontrol grubunda essitalopram uygulamas›na anlaml› yan›t bulunamad›. Bazal dehidroepiandrosteron-sülfat seviyesinin panik bozuklu¤u ve yayg›n anksiyete bozuklu¤u olan hastalarda düflük oluflu, bu hormonun bu hastal›klar›n patofizyolojisinde bir rol oynuyor olabilece¤ini düflündürmektedir.
