Objective: It has been demonstrated that children with Attention Deficit Hyperactivity Disorder (ADHD) have lower levels of self-esteem compared to healthy controls. There is paucity of data regarding the impact of stimulants on the self esteem of children with ADHD. We aimed to see whether methylphenidate had a profound effect on the self-esteem of children with ADHD.

Method : Thirty-one children with a DSM-IV diagnosis of ADHD and who were on methylphenidate for 4-6 months were compared to 40 drug-free controls with a DSM-IV diagnosis of ADHD on the measures of self-esteem. Piers-Harris Self-esteem Scale for Children was used to assess self-esteem in the study and control groups.

Results: There was a statistically significant difference on the measures of physical appearence and school performance/mental state subscales of the self-esteem scale. Subjects who were on methylphenidate had higher self-esteem on these peculiar areas. They also scored significantly lower on the anxiety measure of the scale compared to drug-free controls. Despite these findings, no significant difference was observed in the total scores of the self-esteem scale.

Conclusions: Our study, in line with the previous studies, shows that ADHD children who are on methylphenidate perceive their school performance more positively than those subjects who do not use methylphenidate. We understand that methyphenidate has a positive impact on the self-esteem of ADHD children by recovering the attention deficit and cognitive problems and thereby enhancing the school performance.


Dikkat eksikliği hiperaktivite bozukluğunda metilfenidat ve benlik saygısı

Amaç: Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) olan çocuklarda benlik saygısının düşük olduğu gösterilmiştir. Stimülanların DEHB olan çocukların benlik saygısı üzerine etkisini araştıran araştırmalar az sayıdadır. Çalışmamızın amacı metilfenidatın DEHB olan çocuklarda benlik saygısını ne ölçüde etkilediğini araştırmaktı.

Yöntem: DEHB tanısıyla izlenip 4-6 ay süreyle metilfenidat kullanan 9-14 yaşlar arasıda 31 çocuk ile ayn yaş grubunda ilaç kullanmayan 40 çocuğa Piers-Harris’in çocuklarda öz Kavramı ölçeği (PH??K?) uygulanmış ve iki grubun benlik saygyı ölçümleri karşılaştırmıştır.

Sonuçlar: Metilfenidat kullanan deneklerin fiziksel görünüm ve okul durumu/zihinsel durum alt ölçek puanları ,metilfenidat kullanmayan deneklere oranla istatistiksel olarak anlamlı derecede yüksek bulunmuştur. Metilfenidat kullanan çocukların kaygı alt ölçek puanları ise kullanmayan çocuklara oranla istatistiksel olarak anlamlı derecede düşüktü. İki grubun PHÇÖKÖ toplam puanlar arasıda istatistiksel olarak anlamlı fark bulunmamıştır.

Tartışma: Çalışmamızda benzer çalışmalarda olduğu gibi metilfenidat kullanan olguları akademik beceri alanıda kendilerini daha olumlu algıladıkları tespit edilmiştir. Bu sonuç metilfenidatın dikkat bozukluğunu gidererek çocuğun bilişsel yetilerinde ve okul performansında düzelme sağladığı ve bölece benlik saygısı üzerinde olumlu etki yaptığını düşündürmektedir.
