Telogen effluvium is one of the common causes of diffuse hair loss and abnormality of hair cycling that results in excessive loss of telogen hairs. Emotional stress is attributed to telogen effluvium. Treatment should be focused on the cause. In this article, we presented a case that has not recovered from Telogen effluvium by treatment in dermatology clinic for about 4 years unless she was admitted the psychiatry clinic and used sertraline. A 25 year-old, single female patient admitted to our psychiatry clinic complaint with hair loss. She has had hair loss problem for 5 years and had been diagnosed as Telogen effluvium with trichogram and treatment in dermatology clinic. Then she was adviced to visit psychiatry clinic, since she was not getting any benefit out of the treatment that was given in dermatology clinic. Her speaking speed decreased, mood was depressive, thought content was specifically related to hair loss. The patient has got to live alone and away from her family for some time and hair loss problem started concurrently. A comprehensive biochemical profiling, blood counts, thyroid function test results were all in normal ranges. Sertraline was started 50 mg/day and when she came to the control one month later, she expressed decline in her hair loss. In 3rd and 4th controls hair loss has remitted completely. After sertraline treatment, her hair loss complaint stopped. It is discussed if that was time-dependent. If it was time-dependent it can be thought that the recovery would be for a while and happen in long time. There are very rarely seen cases reported that sertraline caused hair loss, according to our knowledge. There is no reported case of hair loss associated with successfully ended sertraline treatment in the literature. Hair loss type may play significant role. As a result, it should be considered that psychopharmacological treatment is an important choice in hair loss with emotional stress.
Telogen efşuvium tedavisinde sertralin kullanımı: Bir olgu sunumu
Telogen efşuvium, yaygın saç dökülmesinin sık görülen nedenlerinden biridir ve anormal saç siklusunda telogen fazdaki saçların aşırı dökülmesiyle karakterizedir. Emosyonel stres, telogen efşuvium ile sıklıkla ilişkilidir. Tedavisi altta yatan nedene yönelik olmalıdır. Bu yazıda 4 yıldır dermatoloji kliniğine giden ama tedavilerden fayda görmeyen ancak sertralin tedavisinden faydalanan bir telogen efşuvium vakası sunulmuştur. 25 yaşında, bekar, kadın hasta psikiyatri polikliniğimize saç dökülmesi şikayetiyle başvurdu. 5 yıldır saç dökülmesi olduğu, dermatoloji polikliniğinde takip edildiği ve trikogram yapılarak telogen efşuvium tanısı aldığı öğrenildi. Dermatoloji polikliniğinde kendisine verilen tedavilerden fayda görmediği ve diğer sebeplerin dışlanarak psikiyatri polikliniğine gelmesi tavsiye edilmişdi. Ruhsal durum muayenesinde; konuşma miktarı azalmış, afekti depresif, düşünce içeriğinde saçının dökülmesiyle ilgili yoğun düşünceleri vardı. Hasta bir dönem ailesinden ayrı ve yalnız yaşamak zorunda kaldığını ve bu sırada saç dökülmesinin başladığını ifade ediyordu. Hastanın hematolojik ve biyokimyasal değerleri normal sınırlardaydı. Hastaya sertralin 50 mg/gün başlandı ve başlandıktan sonraki ay hasta polikliniğe geldiğinde saç dökülmesinin azaldığını ifade etti. 3. ve 4. kontrollerinde ise tamamen durduğu görüldü. Sertralin tedavisinden sonra hastanın saç dökülmesi şikayeti durmuştur. Ancak bunun zamansal bir süreç mi olduğu tartışma konusudur. Zamana bağlı olsaydı düzelmenin belli bir oranda ve uzun sürede gerçekleşeceği düşünülebilir. Literatürde nadir de olsa sertralinin neden olduğu saç dökülmesi olguları bildirilmiş olsa da bildiğimiz kadarıyla sertralin tedavisinden fayda gören saç dökülmesi olgusu yoktur. Bunda saç dökülmesinin tipi önemlidir. Sonuç olarak emosyonel stresin bulunduğu saç dökülmesi olgularında psikofarmakolojik tedavinin önemli olduğu akılda tutulmalıdır.
